I was an adult. A friend of mine had rented out a small theatre and was showing a couple of his favorite horror movies and calling it a Halloween Party.
I did the skull-face with makeup and I used a pruning saw for the scythe. It had an adjustable curved blade, so I set it at 90 degrees. It was scary looking because the blade was kinda rusty. The theater had the one guy there to keep an eye on things. We showed him our invitations and then he said, "Wow, is that REAL?!?"
I should have had my wife go as Gaiman's version of Death. Ah, hindsight.

Death
The Mirror from Snow White
The Mirror from American McGee's Alice
Whatsherface's Door from Monsters Inc.
There have been other things, but they don't matter.
